If someone got new glasses, they'd like you to say: nice glasses
if someone got a new hairstyle, they'd like people to comment on that.
So, assuming she (and assuming it's a she) wasn't forced to do it or had to do it for medical reasons, she would probably like to be complimented on the result.
thus: "I like the way your breast fill up your shirt more then before" would do.
or:
wow! those bigger boobs sure make your tummy look smaller!
if someone got a new hairstyle, they'd like people to comment on that.

I think it depends on your relationship with the new breastee:
If it's a professional relationship, any comment would be highly inappropriate.
If the person is a close friend or is very open regarding the operation, you could feel confident making a light-hearted observation, or genuine compliment.
If you now find this person more attractive and want to hump her, again, a comment would be rather unsavoury and will not lead to you performing the nudey lying-down dance with her.

it's actually both younger sisters of a good friend of mine... so i've known them for years but am not close friends with either of them.
i used to have a thing for my friend, and when her sisters were old enough they started catching my eye too.... and now with their new additions...... bad thoughts bad thoughts bad thoughts
